The model below shows health factor conditions which lead to health outcomes. Some health factors, such as race and age, cannot be modified but are included in the model because they play a role in individual and population health outcomes. Many health factors, on the other hand, can be modified. One of the roles of public health practitioners is to work with individuals and communities to improve modifiable health factors so that health outcomes will improve. In addition they help inform policy and policy makers and can be used to promote accountability among governmental and non-governmental agencies.
